About The Position

The Wellness Nurse supports residents’ health and well-being through ongoing clinical monitoring and support. The role assists community nurses, caregivers, and medication personnel in maintaining regulatory compliance and clinical excellence. It requires effective teamwork, flexibility, adaptability to changing duties and pace, and a warm, encouraging disposition.

The position is part-time on the overnight shift.

Requirements

• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration as an LPN or RN.
• Working knowledge of state regulations, policies, and procedures required for compliance.
• Ability to work effectively as part of a team, with flexibility and adaptability to changing duties and pace.
• A warm and encouraging disposition, eagerness to learn, and compassion for residents.

Nice To Haves

• Three years of experience in assessment and geriatric nursing preferred.

Responsibilities

• Assist in coordinating resident admissions, including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer medications and treatments, ensuring medications are administered according to physicians’ orders.
• Help coordinate care and services with outside providers, including resident physicians.
• Document and report care provided and resident observations, including unusual or significant changes.
• Maintain working knowledge of state regulations, policies, and procedures.
