About The Position

The Wellness Nurse supports residents’ health and well-being through ongoing clinical monitoring and support. The role assists community nurses, caregivers, and medication personnel in maintaining regulatory compliance and clinical excellence. Success requires working effectively as part of a team, adapting to changes in duties and pace, and maintaining a warm, encouraging disposition.

This is a part-time position scheduled from 7 a.m. to 7 p.m. Compensation is $26.00–$30.25 per hour.

Requirements

• Working knowledge of state regulations, policies, and procedures applicable to compliance.
• Graduation from an accredited school of practical nursing.
• Current nursing license and state registration as an LPN or RN.
• A smile, eagerness to learn, and compassion for residents.

Nice To Haves

• Three years of experience in assessment and geriatric nursing.

Responsibilities

• Assist with coordinating resident admissions, including screening, assessment, and identification of care needs.
• Ensure care is delivered according to resident care plans.
• Administer medications and treatments, ensuring medications are given in accordance with physicians’ orders.
• Help coordinate care and services with outside providers, including residents’ physicians.
• Document and report care provided and resident observations, including unusual or significant changes.
• Maintain working knowledge of applicable state regulations, policies, and procedures.
