About The Position

The Wellness Nurse supports residents’ health and well-being through ongoing clinical monitoring and support. The role assists community nurses, caregivers, and medication personnel in promoting regulatory compliance and clinical excellence. Success in the position requires working effectively as part of a team, adapting to changes in duties and pace, and maintaining a warm and encouraging disposition.

Requirements

• Working knowledge of state regulations, policies, and procedures relevant to compliance.
• Graduate of an accredited school of practical nursing.
• Current nursing license and state registration as an LPN or RN.
• Ability to work effectively as part of a team and adapt to changes in duties and pace.
• Warm and encouraging disposition, eagerness to learn, and compassion for residents.

Nice To Haves

• Three years of experience in assessment and geriatric nursing is preferred.

Responsibilities

• Assist with coordinating resident admissions, including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer medications and treatments, ensuring medications are given according to physicians’ orders.
• Assist in coordinating care and services with outside providers, including resident physicians.
• Document and report care provided and resident observations, including unusual or significant changes.
• Maintain working knowledge of applicable state regulations, policies, and procedures.
