About The Position

At Sunrise Senior Living, the Wellness Nurse (LPN/LVN) supports resident health, monitors care needs, and ensures safe medication practices and clinical compliance. The nurse conducts wellness assessments, monitors changes in residents’ health, supports medication management, communicates with families, physicians, and care teams, and contributes to infection control and clinical quality initiatives. The role also includes training and supporting care team members.

Requirements

• Active LPN/LVN license.
• At least 1 year of experience in a senior living or clinical setting.
• Strong assessment and documentation skills.
• Knowledge of medication administration and compliance.
• Strong communication and problem-solving skills.

Responsibilities

• Conduct wellness assessments and monitor changes in residents’ health.
• Support medication management and perform MAR/TAR audits.
• Communicate with families, physicians, and care teams.
• Monitor incidents and falls and ensure follow-up care.
• Maintain documentation and comply with regulations.
• Support infection control and clinical quality initiatives.
• Train and support care team members.
