About The Position

At Sunrise Senior Living, Wellness Nurses (LPN/LVN) support resident health, monitor care needs, and help ensure safe medication practices and clinical compliance. The role includes conducting wellness assessments, monitoring changes in residents’ health, supporting medication management, communicating with families and care teams, maintaining clinical documentation, and supporting infection control and quality initiatives. The nurse also trains and supports care team members.

Requirements

• Active LPN/LVN license.
• At least 1 year of experience in a senior living or clinical setting.
• Strong assessment and documentation skills.
• Knowledge of medication administration and compliance.
• Strong communication and problem-solving skills.

Responsibilities

• Conduct wellness assessments and monitor changes in residents’ health.
• Support medication management and perform MAR/TAR audits.
• Communicate with families, physicians, and care teams.
• Monitor incidents and falls and coordinate follow-up care.
• Maintain documentation and comply with regulations.
• Support infection control and clinical quality initiatives.
• Train and support care team members.
