About The Position

The Wellness Nurse supports residents’ health and well-being through ongoing clinical monitoring and support. The role assists community nurses, caregivers, and medication personnel in maintaining regulatory compliance and clinical excellence. It requires effective teamwork, flexibility, adaptability to changing duties and pace, and a warm, encouraging disposition. This is a part-time position scheduled from 2–10 p.m. every other weekend.

Requirements

• Graduate from an accredited school of practical nursing.
• Hold a current nursing license and state registration as an LPN or RN.
• Working knowledge of state regulations, policies, and procedures relevant to compliance.
• Work effectively as part of a team, with flexibility and adaptability.
• Bring a warm, encouraging disposition, eagerness to learn, and compassion for residents.

Nice To Haves

• Three years of experience in assessment and geriatric nursing is preferred.

Responsibilities

• Assist with coordinating resident admissions, including screening, assessment, and identifying care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer medications and treatments and ensure medications are administered according to physicians’ orders.
• Help coordinate care and services with outside providers, including resident physicians.
• Document and report care provided and resident observations, including unusual or significant changes.
• Maintain working knowledge of applicable state regulations, policies, and procedures.
