About The Position

Bridge Senior Living is hiring a part-time Wellness Nurse. The nurse implements and coordinates care delivery in collaboration with physicians and care associates, while contributing to a positive environment for residents. The role includes assessing, planning, implementing, and evaluating resident care; making frequent rounds; documenting pertinent care information; and observing residents’ overall condition and behavior. The Wellness Nurse also assists the Director of Wellness with developing and updating resident care plans and communicates with residents, families, and physicians about resident care. Weekend availability is required.

Requirements

• High school diploma or equivalent (GED).
• LPN license in the applicable state.
• First Aid and CPR certification.
• At least 1 year of related work experience.
• Weekend availability.

Responsibilities

• Implement and coordinate the delivery of care in collaboration with physicians and care associates.
• Assess, plan, implement, and evaluate residents’ care.
• Make frequent rounds to provide a high level of care and ensure continuity of care.
• Document pertinent care information and observe residents’ overall condition and behavior.
• Assist the Director of Wellness in developing and updating resident care plans.
• Maintain ongoing communication with residents, families, and physicians regarding resident care.
