About The Position

The Wellness Nurse supports residents’ health and well-being through ongoing clinical monitoring and support. The role assists community nurses, caregivers, and medication personnel in maintaining regulatory compliance and clinical excellence. Success in this position calls for effective teamwork, flexibility and adaptability to changing duties and pace, and a warm, encouraging disposition. This is a full-time position scheduled from 7 a.m. to 7 p.m., with every other weekend required. Compensation is $26.00–$30.55 per hour.

Requirements

Graduate from an accredited school of practical nursing.
Current nursing license and state registration as an LPN or RN.
Working knowledge of applicable state regulations, policies, and procedures for compliance.
Ability to work effectively as part of a team, with flexibility and adaptability to changing duties and pace.
A warm, encouraging disposition, eagerness to learn, and compassion for residents.

Nice To Haves

Three years of experience in assessment and geriatric nursing is preferred.

Responsibilities

Assist with coordinating resident admissions, including screening, assessment, and identification of care needs.
Ensure care is delivered as indicated in resident care plans.
Administer medications and treatments and ensure medications are administered according to physicians’ orders.
Assist with coordinating care and services with outside providers, including resident physicians.
Document and report care provided and resident observations, including unusual or significant changes.
Maintain working knowledge of applicable state regulations, policies, and procedures.
