About The Position

Moments Hospice is seeking a part-time, field-based Weekend Registered Nurse to provide hospice care on Saturdays and Sundays from 8:00 AM to 8:00 PM. The nurse serves as the clinical lead for assigned patients and their families, managing symptoms, coordinating care, and providing support and guidance during end-of-life care. Requirements

• Active Registered Nurse (RN) license in the state where the nurse works.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver’s license and a reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients’ physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plans of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers about disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across an assigned caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
