About The Position

St. Croix Hospice is seeking a compassionate Weekend Registered Nurse to provide care within its 24/7 hospice care model. The nurse responds to urgent needs, makes after-hours visits, completes admissions, and provides clinical expertise and compassionate support to patients and families navigating end-of-life care. The position is based in Troy, Michigan, with 100% travel. The schedule is Friday, Saturday, and Sunday, with 10- or 12-hour days, and is designated as full-time.

Requirements

• Graduate of an accredited nursing program; an associate, bachelor’s, or master’s degree is accepted.
• At least 1 year of recent acute care experience.
• Current RN license in the state of practice.
• CPR certification.
• Reliable transportation, a valid driver’s license, and proof of auto insurance.

Nice To Haves

• Hospice or home care experience.

Responsibilities

• Respond to triage calls and provide after-hours nursing visits for symptom management, admissions, and care plan updates.
• Develop and adjust individualized care plans based on patients’ evolving needs.
• Administer prescribed medications and treatments, and provide education and support to patients and caregivers.
• Maintain timely and accurate clinical documentation of care, outcomes, and communication.
