About The Position
Alpha Home Health and Hospice is a Medicare 5-Star rated agency committed to exceptional care, ethical practice, and our Core Values of CAPLICO. We are seeking a Weekend SOC Admissions RN to support our growing Home Health team in Snohomish County. This PRN position offers flexibility and compensation per completed Start-of-Care (SOC) admission.
Requirements
- Current WA RN license
- At least 1 year experience in a Home Health agency as an RN
- Working knowledge of OASIS
- Current AHA BLS
- Strong assessment and independent practice skills
- Reliable transportation
Responsibilities
- Complete weekend Start-of-Care admissions
- Perform comprehensive assessments and OASIS documentation
- Establish the initial Plan of Care
- Educate patients and families
- Coordinate with physicians and the interdisciplinary team
