About The Position

The VNA of the Treasure Coast is a mission-minded non-profit organization that has been committed to making a difference in the community we serve since 1975. They provide compassionate, innovative care of the highest quality, setting the standard for patients and caregivers needing private care, home health, hospice, and community health services. The Visiting Nurse Association of the Treasure Coast is seeking a Weekend Admissions RN to assume responsibility for providing skilled nursing care and health teaching to patients and their significant others in the home setting.

Requirements

  • Must be currently licensed as an RN in the state of Florida.
  • Minimum of one year full-time experience in a Medicare home health environment.
  • Must be proficient in OASIS.
  • Must have two years of home health experience to qualify for sign-on bonus.
  • This position requires screening through the Care Provider Background Screening Clearinghouse.

Responsibilities

  • Administers medications and treatments as prescribed by the physician. Observes patient condition to monitor response to treatment.
  • Counsels and teaches patient and significant others to accomplish goals.
  • Conducts physical assessment and history to determine health problems and determines intervention required.
  • Develops Plans of Care, conducts initial assessments, follows a care plan which addresses problems requiring intervention.
  • Assesses patient and family for appropriateness of and acceptance for home care.
  • Serves on agency committees as requested.
  • Assists in orientation of new staff nurses.
  • Provides supervision to Home Health Aides and prepares their assignments.
  • Participates in community health projects.
  • Maintains clinical records according to agency, State and Federal guidelines.
  • Participates in QA activities as assigned.
  • Understands medico-legal implications of records and recordings.
  • Determines the focus of each patient visit.
  • Refers to other resources within the agency and to appropriate other community agencies as needed.
  • Recognizes families' abilities, limitations, and stresses, and helps them cope with their changing roles.
  • Coordinates services of interdisciplinary team.
  • Formulates appropriate discharge plans.
  • May participate in agency in-service education programs.
  • Assists with flow of communication from weekend staff to weekday staff.
  • Works one summer and 1 winter holiday each year.
  • Manages on-call duties as part of the primary care model to best serve clients.
  • Participate and support the VNA Disaster Emergency Plans, including updating contact information for agency, manager and department.
  • Inquire, and follow department disaster plan and support any contingency plan as needed.
  • Participate in department debrief of incidents and help identify corrective actions if necessary.
