About The Position

The Waiver Specialist supports people seeking access to the Community Supports Waiver (CSW) and assists people with informed provider selection for both the Intellectual and Developmental Disabilities (IDD) and CSW waivers. The role conducts eligibility assessments, prepares and submits documentation, communicates with individuals and their support networks, and collaborates with CDDO colleagues to complete organizational functions. The position also participates in eligibility forums and community education about CSW services and requirements.

Requirements

Bachelor's degree in Human Services or a related field.
One year of Intellectual and Developmental Disability (IDD) experience.

Nice To Haves

Two years of experience working with the IDD population.
Targeted Case Management experience.

Responsibilities

Facilitate the Community Supports Waiver screening tool assessment with the person and their support team.
Work with KDADS/KDHE to confirm CSW eligibility and complete the Provisional Plan of Care (PPOC) for submission to KDADS approval.
Assist people selecting a new provider or changing an existing provider by completing Freedom of Choice and annual Comprehensive Options Counseling documents.
Review open-provider lists and help people complete the required choice form to select a provider for IDD and CSW waivers.
Regularly update the Freedom of Choice and Comprehensive Options Counseling document listing providers affiliated in Johnson County.
Conduct Medicaid Functional Eligibility Instrument (MFEI) assessments and obtain certification to complete the KDADS functional eligibility tool.
Participate in quarterly CDDO eligibility forums and future CSW community education about program and functional requirements for accessing CSW services.
Communicate with people supported, guardians, family members, and Targeted Case Managers to provide information about IDD services and supports.
Collaborate with other CDDO team members to ensure CDDO functions are completed.
