2026-2027 Volunteer Application - West Hills Middle
About The Position
This is a volunteer application for West Hills Middle for the 2026-2027 school year. Interested individuals can contact the school's front office to express their interest in volunteering. Volunteers who will have significant unsupervised access to children, as determined by the school principal, must complete an annual Volunteer Application. Following the application review, the school will determine if a fingerprint/background check is necessary (at no cost). Volunteers must also complete the "Code of Conduct" training and sign an acknowledgement form before visiting HR for the background check, as required by the Utah Legislature.
Requirements
- Complete the Volunteer Application annually.
- Schedule an appointment with the Human Resource Department.
- Bring a state-issued ID to the Human Resource Department for your scheduled appointment.
- Take the "Code of Conduct" training.
- Sign an acknowledgement form before visiting HR to complete the background check.
