About The Position

Diversicare is seeking a Van Driver who is also a Certified Nurse Assistant (CNA) to transport residents safely to and from medical appointments. The driver accompanies each resident throughout the visit, handles appointment documents and prescriptions, and reports back to the facility’s RCM after returning. Requirements

Be physically able to help ambulatory and non-ambulatory residents transfer between a wheelchair and a vehicle.
Possess a valid state driver’s license.
Be qualified as a Certified Nurse Assistant (CNA).
Have a clean driving record.
Have a clean criminal record.
Hold any other certifications required by the state.

Responsibilities

Ensure each resident’s safety while transporting them to and from a doctor’s office or hospital.
Accompany residents on medical visits and remain with them until the visit is complete.
Sign residents in upon arrival and present their documents to office personnel.
Before leaving the doctor’s office, collect the resident’s documents and any new prescriptions, check that the doctor has completed the doctor’s order form, and obtain appointment cards for any follow-up visits.
After returning to the facility, report to the RCM and be prepared to answer questions about the visit.
Perform other duties as required and assigned.
