About The Position

Erlanger is seeking a Nurse Practitioner or Physician Assistant to join a busy outpatient Urology practice focused on the evaluation and management of stone disease and general urologic conditions. The APP will work collaboratively with board-certified urologists to provide comprehensive patient care, including outpatient consultations, follow-up visits, diagnostic evaluations, patient education, and care coordination. This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices' patient population. Participates in the practice's quality improvement process and performance evaluation.

Requirements

  • Current licensure to practice as a Nurse Practitioner or Physician Assistant in the State of Tennessee
  • Completion from an accredited Nurse Practitioner Program, Family Nurse Practitioner Program, Adult Nurse Practitioner Program or Graduate from an accredited Physician Assistant Program.
  • Demonstrate knowledge of the principles of growth and development over their life span and possess the ability to assess data reflective of the patient's status.
  • Interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs and to provide the care needed as described in the unit's policies and procedures.

Nice To Haves

  • Minimum of three years experience in a physician office setting preferred
  • Certification in areas of specialty preferred.

Responsibilities

  • Provide comprehensive patient care, including outpatient consultations, follow-up visits, diagnostic evaluations, patient education, and care coordination.
  • Work collaboratively with board-certified urologists.
  • Coordinate care for patients.
  • Support patients/families/caregivers in self-management, self-efficacy and behavior change.
  • Manage the practice's patient population.
  • Participate in the practice's quality improvement process and performance evaluation.
  • Develop evidence-based protocols and utilize them to develop an individualized plan of care.
  • Collect, document, and analyze patient data.
  • Collaborate with the health care team in the development and implementation of testing and treatment according to protocols.
  • Participate in development and implementation of health screening programs for adolescents and adults, and develop protocols for identification of high-risk individuals.
  • Participate in community and corporate education on urological topics.
  • Evaluate direct supervisor by direct observation, in a skills lab, or discussed verbally or written.
  • Participate in clinical research & may assume the responsibility of research coordinator including but not limited to education & enrollment in clinical trials.
