About The Position

At Home Harmony is expanding its home-based medical and pharmacy practice to Raleigh, NC, serving aging and homebound adults with complex chronic conditions. The Nurse Practitioner or Physician Assistant will provide primary care in patients’ homes and help lead the local practice expansion, working with Richmond-based leadership to build and implement the clinical model and develop a local team. This role combines clinical care with relationship-building, networking, and clinical operations oversight. The position involves collaboration with an interdisciplinary team of physicians, pharmacists, nurses, medical assistants, phlebotomists, and clinical liaisons, using healthcare technology to support patient outcomes and provider workflows. Care is delivered to older adults in their homes, group homes, and independent living settings, with a schedule combining in-person visits, telehealth, and chronic condition management.
