Requirements
- Associate's Degree in Nursing
- Active MN RN License
- 5+ years of Clinical Nursing experience
- Some experience with patient education and disease management
Responsibilities
- Assess the patient and identified actual or potential health, safety or educational needs
- Uses communication strategies to achieve desirable outcomes
- Evaluates care and the patient's response to interventions and expected outcomes
- Facilitates continuity of care through interdisciplinary collaboration and coordination of appropriate health care services
