The Treatment Services Case Manager is part of the Counselling Services Team. Treatment Case Manager provides culturally responsive case management support to adults through an Indigenous trauma-informed framework that recognizes the importance of holistic wellness, healing, and connection. Grounded in a relationship-based philosophy of care, the program supports individuals in addressing mental health, emotional, behavioural, and psychosocial challenges through counselling, service coordination, advocacy, and access to community resources. Services are delivered in a manner that promotes respect, collaboration, cultural safety, and client-centered care. This role provides direct support to adults accessing clinical counselling services through case management, waitlist support, intake coordination, single-session counselling, and service navigation. The Treatment Services Case Manager coordinates and monitors client engagement throughout the waitlist process, ensuring individuals receive timely, responsive, and culturally grounded support while awaiting ongoing counselling services. Working collaboratively with clients, families, community partners, and multidisciplinary teams, the Treatment Services Case Manager assesses immediate needs, facilitates access to internal and external resources, and develops individualized support plans that address identified goals and priorities. Through brief intervention, counselling support, advocacy, crisis response, and ongoing case management, the role promotes client stability, enhances coping and problem-solving skills, strengthens connections to community supports, and improves access to services. The Treatment Services Case Manager also contributes to service quality and program effectiveness through accurate clinical documentation, participation in case reviews and team consultations, and the ongoing evaluation of processes that support responsive and client-centered care.
