About The Position

The Treatment Services Case Manager is part of the Counselling Services Team. Treatment Case Manager provides culturally responsive case management support to adults through an Indigenous trauma-informed framework that recognizes the importance of holistic wellness, healing, and connection. Grounded in a relationship-based philosophy of care, the program supports individuals in addressing mental health, emotional, behavioural, and psychosocial challenges through counselling, service coordination, advocacy, and access to community resources. Services are delivered in a manner that promotes respect, collaboration, cultural safety, and client-centered care.
This role provides direct support to adults accessing clinical counselling services through case management, waitlist support, intake coordination, single-session counselling, and service navigation. The Treatment Services Case Manager coordinates and monitors client engagement throughout the waitlist process, ensuring individuals receive timely, responsive, and culturally grounded support while awaiting ongoing counselling services. Working collaboratively with clients, families, community partners, and multidisciplinary teams, the Treatment Services Case Manager assesses immediate needs, facilitates access to internal and external resources, and develops individualized support plans that address identified goals and priorities. Through brief intervention, counselling support, advocacy, crisis response, and ongoing case management, the role promotes client stability, enhances coping and problem-solving skills, strengthens connections to community supports, and improves access to services. The Treatment Services Case Manager also contributes to service quality and program effectiveness through accurate clinical documentation, participation in case reviews and team consultations, and the ongoing evaluation of processes that support responsive and client-centered care.

Requirements

• A minimum HBSW is required or a related degree. Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered.
• Registration or eligibility for registration with a relevant regulatory body (e.g., Ontario College of Social Workers and Social Service Workers, College of Registered Psychotherapists of Ontario) is preferred or may be required depending on program area.
• Minimum of two (5) years of experience working directly with children and youth experiencing emotional, behavioural, developmental, or mental health challenges.
• Demonstrated experience conducting intake, screening, and assessments, including identifying risk, determining service needs, and developing client-centered plans.
• Experience providing short-term counselling, single session support, and walk in/brief intervention services.
• Experience managing and maintain a clinical waitlist, including prioritizing cases, completing check in calls, and supporting clients awaiting service.
• Experience providing individual and group programming, behavioural support, and crisis intervention in school, community, or residential settings.
• Experience collaborating with community partners and referring clients to appropriate internal or external services based on identified needs.
• Experience working within an Indigenous organization or community context is strongly preferred.
• Demonstrated experience developing strength-based intervention plans, implementing therapeutic and skill-building activities, and supporting families and caregivers.
• Experience collaborating within a multidisciplinary team that includes counsellors, mental health clinicians, educators, and cultural supports.
• Knowledge and experience in youth engagement, suicide prevention, and cultural and community-based healing approaches.
• Satisfactory Criminal Records Search.
• Ability to work flexible hours and travel within the Thunder Bay District as determined by the Program Manager in relation to service requirements.
• Must possess a Class “G” Driver’s License and have access to a personal vehicle.
• A valid Standard First Aid/CPR Certificate is recommended.
• Must complete and maintain certification in relevant Ministry of Children and Youth Services Initiatives
