Travel Registered Nurse – RN - Hospice - PA West Territory
About The Position
This role involves providing direct care to hospice patients and support to families, while also serving as a floating clinician. The clinician will be temporarily assigned to different hospice branches based on census needs or staffing gaps. This position is for someone energized by new settings, adaptable to change, and passionate about purpose-driven care. The role requires regional travel with potential overnight stays and offers a company fleet car for eligible employees. The territory covers Altoona, Irwin, Meadville, New Castle, Pittsburgh, Somerset, and Wexford in Pennsylvania.
Requirements
- Current, unencumbered RN license in the state of employment
- Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting)
- Current CPR certification
- Valid driver’s license, auto insurance, and reliable transportation

Responsibilities
- Deliver high-quality, compassionate nursing care for hospice patients
- Manage and implement individualized plans of care
- Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)
- Assess and address patients’ physical, emotional, spiritual, and environmental needs
- Accurately document all care provided and ensure compliance with hospice protocols
- Educate families and caregivers on care instructions, medications, and safety
- Participate in the on-call rotation and provide after-hours care as needed
- Collaborate with physicians, referral sources, and the care team
- Assist with referrals, regulatory documentation, and payer compliance
- Travel to various Gentiva hospice locations as assigned to fill urgent needs
