About The Position

This role is for a Hospice RN – Regional Support who will provide direct, compassionate nursing care to hospice patients and support to their families across multiple communities. The position involves serving as a floating clinician, temporarily assigned to different hospice branches based on census needs or staffing gaps. It requires adaptability to new settings and a passion for purpose-driven care. The role typically follows a Monday–Friday schedule with occasional on-call responsibilities. Significant travel is required, which may include overnight stays within a 100-mile radius, frequent overnight travel to multiple states, or regional travel based on assignment needs. A company fleet car is provided for eligible employees. Gentiva is a national leader in hospice care, palliative care, home health care, and advanced illness management, with nearly 600 locations across 38 states, dedicated to providing care in the comfort of patients' homes.

Requirements

  • Current, unencumbered RN license in the state of employment
  • Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting)
  • Current CPR certification
  • Valid driver’s license, auto insurance, and reliable transportation

Responsibilities

  • Deliver high-quality, compassionate nursing care for hospice patients
  • Manage and implement individualized plans of care
  • Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)
  • Assess and address patients’ physical, emotional, spiritual, and environmental needs
  • Accurately document all care provided and ensure compliance with hospice protocols
  • Educate families and caregivers on care instructions, medications, and safety
  • Participate in the on-call rotation and provide after-hours care as needed
  • Collaborate with physicians, referral sources, and the care team
  • Assist with referrals, regulatory documentation, and payer compliance
  • Travel to various Gentiva hospice locations as assigned to fill urgent needs
