About The Position

The Transplant Social Services Consultant reports to the Transplant Clinical Services Manager and may also receive directions from licensed social workers on the patient’s Care Team. In addition to caring for patients, the Social Services Consultant provides supportive services to family and care team members throughout the patient’s assessment and treatment.

Requirements

  • Bachelor’s degree in psychology, social work, or a related field
  • One (1) year of experience in care coordination and/or working with children & families in the medical, educational, and/or mental health settings.

Responsibilities

  • Facilitate patient engagement by conducting in-person and/or virtual meetings, assessing needs, and coordinating appropriate referrals to community-based resources.
  • Ensure the secure and compliant transmission of confidential patient health information to Healthcare providers in accordance with applicable privacy regulations.
  • Maintain, update, and safeguard medical records, including documentation related to Alcoholics Anonymous participation and behavioral therapy services, ensuring accuracy, completeness, and compliance with organizational standards.
  • Present transplant-related information and education to pre-evaluation patients in a group setting, including expectations for the evaluation process and available community-support resources.
  • Other duties as assigned.
