About The Position

CMHA Toronto is seeking Transitional Housing Workers (THWs) to join the Rosemount Integrated Support Program. The THW plays a vital role in supporting individuals living with mental health challenges who may also be experiencing homelessness, housing instability, or involvement with the justice system. Working within a supportive housing environment at Rosemount campus, the THW provides direct, client-centered support that promotes recovery, stability, independence, and meaningful community engagement. This includes assisting clients with daily living skills, wellness activities, and goal-setting, while responding to situations that arise during scheduled shifts, including evenings and overnights. This position works closely with case managers, clinicians, and other support staff to ensure clients receive consistent and compassionate support as they build the skills necessary for long-term housing stability. THWs are essential contributors in helping clients transition toward more independent living with confidence and dignity. Hourly Pay: $30.19 + 4% in lieu of vacation. Requirements

• Degree/Diploma in Social Work, Social Services or related field in human services
• Experience providing community based direct service to adults with mental illness
• The ability to make informed decisions in dynamic situations, and apply sound judgment when supporting clients with complex needs.
• Knowledge of the criminal justice system, concurrent disorders and issues affecting individuals experiencing homelessness
• Familiarity with homeless outreach initiatives and the shelter system
• Good written and verbal communication skills
• Ability to foster and maintain relationships with the corporate sector and community agencies
• Excellent interpersonal and conflict resolution skills
• Experience working with diverse communities
• Candidates will be required to adhere to our COVID-19 policy and provide proof of vaccination.

Responsibilities

• Supporting clients with daily living skills such as meal preparation, personal care, cleaning, and medication reminders
• Support clients through individualized case management, life skills coaching, and service planning to promote successful transitions to independent living.
• Foster a sense of community among residents and promote social inclusion and empowerment.
• Building positive, trusting relationships to better understand and address clients’ needs while maintaining professional boundaries
• Collaborate with external agencies and internal teams to ensure coordinated care.
• Conducting regular unit inspections to ensure health and safety conditions are being kept
• Responding to crisis calls and supporting crisis prevention by ensuring appropriate interventions and follow-ups are provided in a trauma-informed approach
• Maintaining up-to-date and accurate case documentation within the program database, ensuring that client information, progress notes, and service records are consistently recorded.
• Collaborating with colleagues and community partners to coordinate care, share information appropriately, and identify opportunities for service improvement.
