About The Position

We are seeking a Transitional Counselor to join our HARP Residential Program. The Harbor Apartment and Respite Program (HARP) provides supervised housing services in a cluster apartment model for individuals who benefit from 24-hour staff support while living in their own apartments. The program also offers respite services for individuals experiencing increased symptoms or transitioning from a hospital setting who require additional short-term supervision and support. This is a part-time, 16 hours per week position. Schedule: Saturday and Sunday 8am-4pm. Candidates must reside in Connecticut to be eligible for this position.

Requirements

  • Two years of college education or a high school diploma/equivalent with two years of direct care experience.
  • Experience supporting individuals with behavioral health, mental health, or substance use challenges.
  • Valid driver's license with an acceptable driving record and the ability to transport clients.
  • Ability to obtain and maintain CPR/First and AED certification.

Responsibilities

  • Support residents with daily living activities, including personal hygiene, meal preparation, housekeeping, budgeting, and community integration.
  • Assist individuals in developing, implementing, and achieving personalized recovery goals and service plans.
  • Provide advocacy, emotional support, and recovery-focused counseling services.
  • Connect residents with clinical, vocational, educational, and community resources that promote long-term success.
  • Facilitate rehabilitation activities and groups designed to build independent living skills and support recovery.
  • Utilize Motivational Interviewing and Integrated Dual Diagnosis Treatment (IDDT) principles when working with individuals experiencing co-occurring disorders.
  • Provide transportation and accompany residents to appointments and community activities as appropriate.
  • Respond to crises and collaborate with treatment providers, hospitals, and emergency personnel when needed.
  • Maintain accurate documentation, progress notes, communication logs, and electronic health records.
