About The Position

We are seeking a compassionate and dedicated Transitional Counselor to join our Howe Residential Program. The Howe Residential Supervised Housing Program provides quality, recovery-focused services that promote residents' well-being, community integration, and independent living skills. Through individualized support, rehabilitation, education, advocacy, and skill-building services, the program empowers residents to strengthen their abilities, achieve personal goals, and improve their overall quality of life. This is a full-time, 40 hours per week position. Schedule: Monday, Tuesday, Wednesday, Thursday and Friday from 3pm-11pm. This position is designated as Onsite Essential Staff and is required to report to work during inclement weather and other emergency situations. Candidates must reside in Connecticut to be eligible for this position.

Requirements

  • Two years of college education in Human Services, Behavioral Science, or a related field preferred; OR a High School Diploma/GED with 2-4 years of direct care experience.
  • Experience supporting individuals with mental health and co-occurring challenges preferred.
  • Strong communication, organization, documentation, and problem-solving skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Valid Connecticut driver's license with an acceptable driving record and the ability to transport clients as needed.
  • Ability to obtain and maintain CPR, AED, and First Aid certification.

Responsibilities

  • Providing coaching and skill-building in personal hygiene, meal planning, cooking, housekeeping, budgeting, and community living skills.
  • Coordinating care and connecting residents with behavioral health, vocational, educational, healthcare, and community resources.
  • Facilitating rehabilitation and recovery-focused groups and activities.
  • Advocating for residents while helping them establish natural supports and community connections.
  • Supervising self-medication protocols and supporting residents' health and safety needs.
  • Providing crisis intervention and collaborating with emergency services and treatment providers when necessary.
  • Maintaining accurate documentation, progress notes, communication logs, and electronic health records.
  • Transporting clients and supporting community-based activities as needed.
