We are seeking a compassionate and dedicated Transitional Counselor to join our Howe Residential Program, where you'll support adults living with mental health challenges as they work toward recovery, community integration, and greater independence. This is an excellent opportunity for a human services professional who is passionate about behavioral health, recovery-oriented care, and helping individuals achieve their personal goals in a supportive residential setting. These are Per Diem positions and various shifts are available as this is a 24/7 program. This role is designated as Onsite Essential Staff and required to report to work despite inclement weather or other adverse conditions. Candidates must reside in Connecticut to be eligible for this position. About the Program The Howe Residential Supervised Housing Program provides quality, recovery-focused services that promote residents' well-being, community integration, and independent living skills. Through individualized support, rehabilitation, education, advocacy, and skill-building services, the program empowers residents to strengthen their abilities, achieve personal goals, and improve their overall quality of life.

Requirements

  • Two years of college education in Human Services, Behavioral Science, or a related field preferred; OR a High School Diploma/GED with 2-4 years of direct care experience.
  • Experience supporting individuals with mental health and co-occurring challenges preferred.
  • Strong communication, organization, documentation, and problem-solving skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Valid Connecticut driver's license with an acceptable driving record and the ability to transport clients as needed.
  • Ability to obtain and maintain CPR, AED, and First Aid certification.

Responsibilities

  • Supporting residents in developing, implementing, and achieving individualized recovery goals and service plans.
  • Providing coaching and skill-building in personal hygiene, meal planning, cooking, housekeeping, budgeting, and community living skills.
  • Coordinating care and connecting residents with behavioral health, vocational, educational, healthcare, and community resources.
  • Supervising self-medication protocols and supporting residents' health and safety needs.
  • Providing crisis intervention and collaborating with emergency services and treatment providers when necessary.
  • Maintaining accurate documentation, progress notes, communication logs, and electronic health records.
  • Transporting clients and supporting community-based activities as needed.
