The Transition Supervisor provides leadership and oversight for transition and vocational services within assigned programs and service areas. This position coordinates youth transition programming, supports staff development, manages referrals and caseload assignments, ensures quality service delivery, and promotes positive outcomes for individuals served. Working closely with organizational leadership, community partners, schools, families, and stakeholders, the Transition Supervisor helps ensure individuals receive high-quality, person-centered services that support their goals and success.

Requirements

  • High school diploma, GED, or equivalent required.
  • Current valid driver's license, acceptable driving record, and current automobile insurance required.
  • Successful completion of criminal background, driving record, abuse/neglect, and fingerprint screenings.
  • Strong interpersonal skills with the ability to build trust and collaborate with team members, leadership, stakeholders, and community partners.
  • Ability to exercise sound judgment and independent decision-making.
  • Patient, empathetic, and respectful approach when supporting individuals with disabilities and barriers to employment, education, or independent living.
  • Ability to operate a vehicle safely and travel as necessary.
  • Strong computer proficiency and data entry skills.
  • Ability to maintain confidentiality and adhere to professional ethics, organizational policies, and codes of conduct.
  • Positive attitude and commitment to teamwork and continuous improvement.

Nice To Haves

  • Bachelor's degree in Human Resources, Social Services, or a related field from an accredited institution preferred.
  • Two or more years of relevant experience in vocational services, workforce development, social services, or a related field preferred.
  • Demonstrated ability to organize projects, manage competing priorities, and support program operations.
  • Strong written and verbal communication skills with the ability to prepare reports and present information effectively.

Responsibilities

  • Lead and oversee transition and vocational service programs to support positive outcomes for youth and individuals served.
  • Coordinate transition activities including intakes, onboarding processes, orientation activities, and timesheet management.
  • Support youth in achieving successful community integration, employment readiness, and independence goals.
  • Build and maintain collaborative relationships with families, schools, referral sources, stakeholders, and community partners.
  • Assess individual strengths, interests, abilities, and support needs to guide service planning and delivery.
  • Supervise, coach, and support Transition Specialists and Summer Employment Specialists to ensure quality services and professional growth.
  • Manage program referrals, coordinate caseload assignments, participate in hiring activities, and recommend staffing needs.
  • Monitor quality assurance standards, confidentiality requirements, contractual obligations, CARF standards, and organizational best practices.
  • Oversee accurate billing, reimbursement documentation, database entry, revenue tracking, and expense reporting.
  • Participate in networking, outreach, marketing, and partnership-building activities that expand community awareness of services.
  • Provide direct services and maintain a caseload when necessary to support operational needs.
  • Complete incident reporting and respond to requests for change within established timelines.
  • Perform other duties as assigned.
