About The Position

The Transition & Quality Specialist is responsible for ensuring compliance with state and local regulations and federal accreditation standards. They will also play a crucial role in facilitating smooth transitions for people we support and manage the transition of caseloads and homes. This position will serve as a liaison between landlords and Sunnyhill, Inc. ensuring all parties are well-informed and coordinated. Transition & Quality Specialist will have strong organizational skills, excellent communication abilities, and a passion for supporting the people we support and their teams through all transition processes.

Requirements

  • Three (3) years of experience in the field of developmental disabilities.
  • Excellent verbal and written communication skills.
  • Ability to communicate positively and effectively with people supported, employees and all involved team members.
  • Ability to prioritize tasks and function effectively in a high-paced environment.
  • Valid driver’s license and auto insurance (use of personal vehicle required at times).
  • Acceptable background checks.

Nice To Haves

  • Bachelor’s degree in Social Services, or related field, preferred.
  • Experience providing services funded through the Department of Mental Health, preferred.

Responsibilities

  • Review and ensure compliance with monthly documentation for all residential sites in the Jefferson County Supported Living Program.
  • Complete spot checks of daily log notes for all residential sites to ensure compliance and quality. Provide feedback to team when corrections are needed.
  • Prepare and submit documentation for identified restrictions in REDCap for review, while maintaining the restriction spreadsheet, ensuring timely follow-up, and coordinating the annual review of existing restrictions.
  • Review finalized ISPs for all people supported, including identifying unaddressed restrictions, entering or updating all outcomes and implementation strategies in Therap, and assisting with compliance of annual assessments and consents.
  • Assist with compliance of monthly safety talk data and emergency drill completion.
  • Review the consumer referral database to identify and evaluate potential new people we support for the department and maintain a robust system for tracking referrals and intakes.
  • Act as the agency liaison during meet and greets and throughout the intake process.
  • Ensure completion and ongoing maintenance of all intakes and their supporting documentation.
  • Conduct monthly post-intake meetings with the team for one-year post-transition.
  • Coordinate all move-related activities within the department.
  • Identify, tour, and secure properties that align with the needs of the people we support.
  • Ensure compliance with documentation for rental property leases.
  • Serve as the liaison between Sunnyhill and landlords, ensuring all services outlined in the lease are effectively set up and completed.
  • Complete pre-lease renewal site checks and communicate concerns to management.
  • Assist with preparing annual assessment and consent packets while ensuring all related paperwork remains in compliance.
  • Complete satisfaction surveys with people supported every six (6) months and facilitate resolutions to issues that arise.
