As a center of excellence, we cultivate talent and provide professional purpose. Together we facilitate healing, one patient at a time. We are looking for a Transition Specialist! At Parkside, our transition specialists are discharge planners who coordinate discharge and aftercare planning by securing outpatient appointments with families and providers who can meet the individual patients' needs. They make direct contact with our patients' families and continue contact with them after discharge in an effort to provide warm hand offs to their outpatient providers. Additionally, they help to troubleshoot patients' barriers to attending outpatient services. Our transition specialists take an active role in treatment team meetings, providing guidance regarding community resource options.

Requirements

  • Bachelor's Degree
  • 2 years' experience in the mental health field

Responsibilities

  • Utilize 3rd party pay source resources to locate in-network providers.
  • Schedules out-patient appointments and medication management appointments, coordinating times and dates with families
  • Enters discharge planning notes in EHR
  • Tracks court report due dates and alerts clinicians
  • Participates in treatment team meetings, providing information regarding discharge planning and resources
  • Faxes information to out-patient providers, DHS, court, and others as needed.
  • Functions as a patient and family advocate at all times
  • Maintains and respects confidentiality of patient/physician/personnel information
  • Functions as an integral part of the treatment team
  • Is a liaison between patients, referral sources, agencies, schools, etc.
  • Engages in timely, efficient discharge planning. Has excellent knowledge of referral/placement process and available community resources
  • Keeps current with community resources
  • Maintains a good working relationship within the department and with other departments
  • Communicates appropriately and clearly to patients, families, and team members. Treats everyone with respect and dignity
  • Documentation meets current standards and policies
  • Maintains fit for duty. Acts in a professional manner and follows all of Parkside policies and procedures
  • Orients new staff members to the unit
  • Demonstrates the ability to be organized and flexible; acts appropriately in stressful/emergency situations
  • Able to provide Handle with Care when needed
  • Performs other duties as assigned
