About The Position

Project Hope for Women and Children is seeking a Therapist to work with individuals with a substance use disorder. This role involves providing clinical assessments, formulating treatment plans, and conducting various forms of therapy including individual, group, and family therapy. The therapist will also coordinate with patient navigators and external agencies, document client progress, and provide feedback for service improvement.

Responsibilities

  • Provide clinical assessment, formulate treatment plans, and conduct treatment services for individuals with substance use disorders.
  • Conduct individual, group, and family therapy sessions, as well as psycho-educational offerings.
  • Coordinate with patient navigators to make referrals to other helping agencies.
  • Coordinate services with providers, patient navigators, and recovery coaches.
  • Serve as a consultant for external agencies such as courts and law enforcement, as required by federal and state laws.
  • Continuously assess the individual needs of patients and their families, reflecting these needs in staffing, evaluations, treatment plans, and progress notes.
  • Document all reports for client charts and billing documents in a timely manner and in accordance with policy.
  • Provide feedback to the Director and Administration to improve patient flow and outcomes.
  • Serve on various committees and perform other duties as assigned.
