About The Position

The Physical Therapist provides direct patient care in patients’ homes, applying age-specific standards of practice across the patient populations served. The therapist works independently and collaborates with other team members to achieve patient-centered goals. The role supports patients’ mobility, pain management, and quality of life across mid-Michigan. Henry Ford Jackson Hospital At Home offers personalized home health services through an interdisciplinary team, including care after injury or surgery and chronic illness management.

Requirements

Graduate from an accredited Physical Therapy program with a master's or doctoral degree.
At least one year of experience providing physical therapy in a healthcare setting.
Current Michigan licensure for Physical Therapy.
Valid Michigan driver's license and a dependable vehicle insured as required by the State of Michigan.

Nice To Haves

At least two years of experience as a Physical Therapist.
Experience in homecare.

Responsibilities
