About The Position

Residential Home Health and Hospice is hiring a Physical Therapist to provide skilled, patient-centered home health care to patients in the Mount Vernon, Illinois area. The therapist works independently while collaborating with an interdisciplinary team and travels to patients' homes within an assigned territory. Requirements

• Active Physical Therapist license in the state where the employee works.
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care.
• Valid driver's license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.
• Successful completion of a drug screen prior to employment.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations and functional mobility assessments.
• Develop individualized therapy plans to improve patients' strength, balance, and independence.
• Educate patients and caregivers about therapeutic exercises, injury prevention, and safe mobility strategies.
• Think critically and solve problems in real time.
• Complete thorough, compliant EMR documentation.
• Travel to patient homes within the assigned territory.
