About The Position

AHN Healthcare@Home is hiring a Physical Therapist to provide skilled, patient-centered care to patients in their homes in the Waynesburg, Prosperity, and Amity, Pennsylvania area. Requirements

Active Physical Therapist (PT) license in the state where the employee works.
At least one year of experience as a Physical Therapist.
Clinical expertise to deliver quality patient care.
Valid driver’s license.
Ability to spend approximately 20% of the workday driving to and from patient locations.
Successful completion of a pre-employment drug screen, including screening for medical and recreational marijuana.
Legal authorization to work in the United States.

Responsibilities

Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
Perform comprehensive patient evaluations and functional mobility assessments.
Develop individualized therapy plans to improve patients’ strength, balance, and independence.
Educate patients and caregivers about therapeutic exercises, injury prevention, and safe mobility strategies.
Use critical thinking and problem-solving skills in real time.
Complete thorough, compliant electronic medical record (EMR) documentation.
Travel to patient homes within the assigned territory.
