Physical Therapist- Home Health
About The Position
BAYADA Home Health Care is seeking a Physical Therapist to join its Prescott, Arizona team, serving clients throughout Yavapai County, including the Tri City area and extensions to Paulden and Cordes Lake. The role provides home-based physical therapy as part of a multidisciplinary health care team, helping clients remain at home and reach their goals through individualized treatment plans and compassionate, reliable care. The position is described as up to full-time, with a flexible Monday–Friday schedule and pay of $75–$80 under a pay-per-point model. BAYADA provides paid training in the office, in the home, and online, as well as clinical support around the clock. The organization is a 501(c)(3) nonprofit; employees may qualify for Public Service Loan Forgiveness (PSLF).
Requirements
- Current Physical Therapist license in Arizona.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by a school transcript or diploma.
- Solid computer skills.

- Prior experience with electronic medical records (EMR).
Responsibilities
- Perform diagnostic tests and measurements, including assessment of joint mobility and range, transfers, stability, gait, strength, endurance, balance, and safety.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance activities, and other interventions.
- Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
- Continually assess and revise the physical therapy care plan.
- Educate and instruct clients, family members, or other client representatives in rehabilitative care and activities that promote health, safety, and independent living.
- Document observations, interventions, evaluations, patient care management, and services provided using a touch-pad tablet.
