About The Position

Personalized Health Partners provides behavioral health care in long-term care settings through innovative, team-based approaches, serving residents with a range of mental health and neurocognitive needs. The Therapist/Behavioral Health Care Manager provides psychotherapy, group therapy, and collaborative care management to residents. Candidates must have a master’s degree and an applicable social work or counseling license. The role is offered with full-time and part-time options in the Cleveland region, including Chardon, Lorain, and Berea.

Requirements

Master’s degree in social work or counseling from an accredited institution.
Licensure as an LSW, LISW, LPC, or LPCC; the posting also lists LPCC-S, LISW-S, and licensed independent clinical roles in the title.
Experience working with diverse patient populations, including people with chronic severe mental illness and major neurocognitive disorders.
Proficiency using electronic medical records (EMR) systems.
Strong interpersonal and communication skills.
Ability to work collaboratively within a multidisciplinary team.

Responsibilities

Deliver individual and group psychotherapy sessions tailored to residents’ needs.
Coordinate collaborative care management with psychiatrists, primary care providers, and other team members.
Address residents’ mental health needs, including chronic severe mental illness, substance use disorders, depression, and major neurocognitive disorders.
Develop and implement therapeutic interventions using evidence-based modalities.
Maintain detailed and accurate clinical documentation in an electronic medical records (EMR) system.
Conduct initial behavioral health evaluations of residents as needed.
