About The Position

Personalized Health Partners is seeking Social Workers and Counselors to serve as Therapist/Behavioral Health Care Managers in long-term care settings in the Massillon/Canton area. The role provides psychotherapy, group therapy, and collaborative care management to residents with mental health and neurocognitive needs, including chronic severe mental illness, substance use disorders, depression, and major neurocognitive disorders. The position works collaboratively with psychiatrists, primary care providers, and other members of a multidisciplinary team. Candidates must hold a master's degree and a qualifying Ohio license.

Requirements

Hold an LSW, LISW, LPC, or LPCC license; the posting also lists LPCC-S and LISW-S in the position title.
Master's degree in Social Work or Counseling from an accredited institution.
Experience working with diverse patient populations, including people with chronic severe mental illness and major neurocognitive disorders.
Proficiency using electronic medical records (EMR) systems.
Strong interpersonal and communication skills.
Ability to work collaboratively within a multidisciplinary team.

Responsibilities

Deliver individual and group psychotherapy tailored to residents' needs.
Coordinate collaborative care management with psychiatrists, primary care providers, and other team members.
Address residents' mental health needs, including chronic severe mental illness, substance use disorders, depression, and major neurocognitive disorders.
Develop and implement therapeutic interventions using evidence-based modalities.
Maintain detailed and accurate clinical documentation in an electronic medical records (EMR) system.
Conduct initial behavioral health evaluations of residents as needed.
