About The Position

The Team Leader is a Registered Nurse who plays a key leadership role within the interprofessional team. Using HPHA’s Collaborative Care Model and evidence-informed practice, the Team Leader supports the coordination and supervision of safe, high-quality patient care for patients and families. The Team Leader provides clinical leadership, acts as a resource to staff, supports team communication, and contributes to patient flow, safety, and quality initiatives. The role promotes professional practice, collaboration, and service excellence while supporting the operational needs of the Emergency Department and Inpatient Unit.

Requirements

  • Current certificate of registration in good standing with the College of Nurses of Ontario (CNO) (RN)
  • Advanced Cardiac Life Support (ACLS)
  • 12‑lead ECG interpretation course
  • Non-Violent Crisis Intervention (NVCI)
  • Basic Cardiac Arrhythmia course
  • Preceptor Course (HPHA or equivalent), or willingness to obtain within 3 months of hire
  • Minimum 5 years nursing experience in an acute healthcare setting; current stroke and/or telemetry or related experience preferred
  • Demonstrated exceptional clinical competence, critical thinking, and decision-making skills
  • Demonstrated leadership ability, including conflict resolution and team facilitation
  • Strong communication skills (verbal and written) and experience working with patients, families, and other healthcare providers
  • Commitment to ongoing professional development and continuous learning
  • Strong customer service and person- and family-centred care focus
  • Ability to plan, organize, problem-solve, and support activities of self and unit staff
  • Exceptional time-management skills
  • Ability to manage stressful situations and provide direction during high-acuity events
  • Proficiency with Meditech and Microsoft Office applications
  • Demonstrates a strong commitment to regular attendance and punctuality

  • Bachelor of Science in Nursing (BScN) preferred

Responsibilities

  • Develop and coordinate patient care plans to ensure safe, high-quality person-centred care
  • Promote optimal allocation of resources, supporting throughput and process optimization
  • Mentor and coach staff, providing consistent support through structured orientation, onboarding, and mentorship programs; assist with identifying and addressing learning needs
  • Support staff competency and adherence to standards of care through role modelling, auditing, review of RL6 incident reports, and consultation
  • Support quality care initiatives, accreditation readiness, and unit education
  • Promote and maintain collaborative team relationships across all professions
  • Collaborate with internal and external healthcare providers and organizations
  • Facilitate and lead discharge rounds, huddles, and interdisciplinary patient care meetings
  • Support development, implementation, and review of policies, procedures, protocols, and order sets
  • Order supplies and equipment to support clinical and patient needs
  • Support patient flow, escalation processes, and surge planning
  • Participate in emergency preparedness and response as required
