About The Position

Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking a full-time Team Lead, Community Engagement for the Rural Health Transformation Program. The Rural Health Transformation (RHT) Program is a federal grant that empowers states to strengthen rural communities across America by improving healthcare access, quality, and outcomes by transforming the healthcare delivery ecosystem.

Requirements

  • Community Health Worker Certification (CHW) required.
  • Must hold a valid and unrestricted motor vehicle operator’s license and have reliable transportation.
  • Strong PC and office software skills.

Nice To Haves

  • Two – four years post-secondary schooling
  • Bachelor’s degree; Master’s degree preferred
  • Major: Social Work, Health Services, Public Health or Business Management
  • Supervisory experience or demonstrate the traits necessary to effectively supervise CHWs/CNs
  • At least 3 years’ experience working as a CHW with vulnerable populations.
  • 3+ years previous experience in social service, public health, community resource allocation or related experience.
  • Demonstrate commitment to expanding access to care.
  • Trust building traits and problem-solving skills.
  • Excellent written and oral communication skills.
  • Strong interpersonal and group communication skills.
  • Cultural competence.
  • Organizational, management and program development skills.
  • Working knowledge of community resources and human and community service needs of individuals and families in lower socioeconomic levels.

Responsibilities

  • Supervises the day-to-day work of the Community Health Workers (CHWs) and Community Navigators (CNs) including direct supervision, mentoring, support, and training.
  • Monitors staff performance and workload as well as assessment and referral processes.
  • Initiates and maintains positive communication and collaboration with internal stakeholders, providers and other community social service organizations.
  • Functions as a resource for individual contributors and support staff (clinical and non-clinical).
  • Acts as a subject matter expert and ensures consistency in procedures and workflows.
  • Monitors operational reports and team compliance with requirements.
  • Works with the department manager to ensure alignment with staffing, performance management and resource needs.
  • Assists with development and implementation of Community Health Worker Ambassador Program and maintains program materials and supplies.
  • Carries a reduced case load and makes outreach phone calls, home visits, and face to face visits at community engagement HUBs and other settings where members can be found.
  • Manages the referral process to support Social Drivers of Health (SDOH) needs, Care Management (CM) referrals including but not limited to chronic disease cases.
  • Reviews productivity reports and dashboards to help the manager identify trends and areas for improvement.
  • Conducts monthly team, regional and one-on-one meetings with staff.
  • Recommends changes to workflows, processes, and procedures to streamline functions.
  • Generates monthly reports and assures that all standards are met.
  • Represents the team on internal committees, work groups and task forces as assigned.
  • Participates in external statewide partner meetings to support HUSKY Health members.
  • Ensures staff adherence to confidentiality and HIPPA regulations.
  • Manages overall performance and provides progressive discipline and positive recognition as appropriate.
  • Leads, motivates and mentors staff and facilitates problem solving.
  • Identifies and addresses training needs of staff.
  • Manages schedule, tracks attendance and timecard accuracy, and projects staffing for daily and future (PTO) needs.
  • Ensures compliance with quality and safety regulations and standards.
  • Performs other related functions as assigned.
