About The Position

Community Health Network of Connecticut, Inc. (CHNCT) is seeking a full-time Community Health Educator for the Rural Health Transformation Program. This program is a federal grant aimed at strengthening rural communities by improving healthcare access, quality, and outcomes through the transformation of the healthcare delivery ecosystem.

Requirements

  • Four years post-secondary schooling
  • Bachelor's degree required
  • Major in Health services, allied health, nutrition or public health discipline
  • 1-2 years' relevant experience
  • Experience in health education, with knowledge of Medicaid population preferred
  • Experience working in a community-based setting for at least 1-2 years preferred
  • Trust-building traits and problem-solving skills
  • Current and unrestricted motor vehicle operator's license
  • Reliable transportation
  • State Motor Vehicle License

Responsibilities

  • Increasing healthy living behaviors by providing health education using evidence-based programs to members in the HUSKY Health Program at health fairs, community, and school events.
  • Dispensing and collecting evidence-based pre/post surveys and referrals to the Community Health Worker program for Social Drivers of Health (SDOH) and benefit education support.
  • Teaching culturally appropriate health and nutrition information in classroom settings and at community health and wellness events statewide.
  • Conducting statewide research for workshop and exhibiting opportunities, attending public health webinars and meetings, and completing other assigned projects.
  • Cultivating and maintaining awareness of community cultures and values.
  • Coordinating partnerships and opportunities with community organizations in collaboration with the community engagement team.
  • Educating and providing resources to all ages for health conditions such as obesity, hypertension, heart disease, stroke, diabetes, etc.
  • Facilitating Social Drivers of Health (SDOH) and benefit education referrals to the Community Health Worker Ambassador Program.
  • Participating in health fairs, group workshops, meetings, webinars, health promotion, and outreach activities online and in the community.
  • Administering and collecting pre/post assessments during nutritional workshops and completing data entry of surveys within the established timeframe.
  • Completing necessary tasks before and after events, including printing/scanning materials, picking up/dropping off supplies, and office work.
  • Reviewing and responding to emails as needed.
  • Performing other related duties as assigned.
