About The Position

Community Health Network of Connecticut, Inc. (CHNCT) is seeking a Community Engagement Coordinator for the Rural Health Transformation Program. This program is a federal grant aimed at strengthening rural communities by improving healthcare access, quality, and outcomes through healthcare delivery system transformation.

Requirements

  • High school diploma or GED required
  • Current and unrestricted motor vehicle operator's license
  • Reliable transportation
  • HIPAA compliant home workspace
  • Employee supplied high-speed internet connection
  • Company supplied computer workstation and telephone technology
  • Attendance in person for meetings, training, etc., is required.

Nice To Haves

  • Bachelor's degree preferred
  • Experience in Social services, public health, or business management
  • 1-3 years relevant experience
  • Experience in community organizing and managed care
  • Knowledge of Medicaid population preferred
  • Experience working in a community-based setting for at least 1-2 years preferred
  • Community Health Worker (CHW) certification preferred
  • Mental Health First Aid (MHFA) certification preferred

Responsibilities

  • Manages internal and external reporting processes and oversees the Community Engagement dashboard.
  • Actively participates in Community Engagement meetings, providing insights on report development and sharing key findings.
  • Serves as a community liaison with specialized knowledge in reducing barriers to accessing care and community resources.
  • Interfaces with community engagement HUB sites, state and local nonprofit organizations to expand the Community Engagement HUB Program.
  • Organizes policy and procedure postings through SharePoint and maintains efficiency of secured documents on Community Affairs drive.
  • Assists with identified administrative team functions.
  • Accountable for timely development of assigned reports and dissemination to appropriate teams.
  • Monitors department resources for inventory maintenance, data analytic reports and any other identified reports.
  • Produces and formats reports needed for presentations and creates PowerPoints presentations for meetings as requested by the Director.
  • Manages all Memorandum of Understanding (MOU) and Certificate of Insurance (COi) files.
  • In collaboration with internal and external partners, coordinates staffing for HUBs, health fairs, community meetings, and health promotion and outreach activities.
  • Coordinates with Corporate Communications department to ensure HUB and event dates are posted monthly on the HUSKY Health member portal events calendar.
  • Support community affair initiatives, corporate events and other community activities as needed.
  • Performs other related functions as assigned.
  • Gathers feedback from community HUB partners on staff's effectiveness and experience working with CHNCT.
  • Participate in meetings to discuss HUB program expansion and staff management at HUB sites.
