About The Position

CarePartners is seeking a Team Coordinator to ensure effective customer service and communication between patients, families, and staff. The Team Coordinator will be responsible for scheduling PSWs, Nurses, and Therapists to meet service delivery standards and perform administrative functions related to client services in a timely manner. This is a Full-Time temporary opening for a 6-month contract in the Peterborough office. The hours of work involve a schedule of defined shifts, including days, afternoons, evenings, and weekend shifts. This position is part of a Collective Bargaining unit.

Requirements

  • A strong desire and commitment to making a difference in the lives of their patients.
  • One (1) year of job-related experience (such as administrative or customer service) is required.
  • Exceptional interpersonal and customer service skills.
  • Ability to work both independently and as part of a team.
  • Strong proficiency in all aspects of Microsoft Office, with proven skills in Word and Excel.
  • A strong attention to detail and the ability to learn new systems and databases.
  • Effective verbal and written communication skills.
  • Non-traditional hours, including evenings and weekends are required.
  • A clear background check.

Nice To Haves

  • Post-secondary education is preferred.
  • Experience working within a unionized environment.
  • Experience working in the health care field.

Responsibilities

  • Act as the bridge between patients/caregivers, their families, Personal Support Workers, team supervisors, funders, finance and other CarePartners employees.
  • Receive routine referral/intake information from funders and private-pay clients.
  • Develop and coordinate service schedules considering relevant factors.
  • Inform workers of new assignments and changes to their schedules in accordance with the collective agreement and seniority.
  • Enter relevant information into the automated information system/database.
  • Produce schedules and reports on a regular basis, and upon request of the Supervisor.
  • Receive and manage inbound telephone calls and email inquiries related to service.
  • Perform other administrative duties.
  • Perform other duties as required.
