Requirements

Bachelor’s degree in social work, education, psychology, or another related field, OR a High School diploma with at least three years working in the human services field
Experience working with people with mental health conditions
Valid car insurance
Valid driver’s license and insurable driving record

Nice To Haves

Two years of experience in a case management or care coordination role preferred
Experience working with people with autism spectrum disorders and developmental disabilities preferred
Experience working in a school-based setting preferred

Responsibilities

Providing behavioral health treatment services to youth with co-occurring behavioral health and developmental disabilities to assist supporting their behavioral health needs and tracking relevant health outcomes.
Supporting skill development and symptom management in residential and community settings.
Providing service linkage both within Boundless and with outside providers that identify and address behavioral health needs, developmental issues and barriers to care.
Working with individuals, parents/guardians, Boundless staff and outside providers that support treatment and contribute meaningfully to whole health outcome of the individual served.
