About The Position

Coastal Horizons Center is seeking a TASC Care Manager to provide case management and service coordination for justice-involved individuals in Wilson County, North Carolina. The position supports participants in accessing treatment and recovery services through collaboration with community partners, justice systems, and service providers. Primary duties include administering psychosocial screenings and assessments, coordinating connections to behavioral health treatment and medical, educational, and vocational services as needed, and providing ongoing professional care management.

Requirements

Bachelor’s degree or higher in a human services or related field.
Knowledge of recovery, harm reduction, and wellness-based concepts.
Strong communication, organizational, and time-management skills.
Ability to work independently and manage a caseload effectively.
Proficiency in Microsoft Office Suite and similar platforms.
Proficiency in electronic health record systems and remote collaboration tools such as Teams and Zoom.
Demonstrated ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services.

Nice To Haves

North Carolina Peer Support Specialist certification and at least two years of experience.
Registration with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB).
Lived experience with recovery and/or the justice system.

Responsibilities
