About The Position

This is a full-time position requiring 40 hours per week. The schedule will be determined by a supervisor to meet location and departmental needs, and may vary depending on company needs and workload. Employees will be given as much notice as possible when changes occur. Travel is minimal. The role involves working with mental health or substance use populations and requires a high school diploma or equivalent.

  • Work with mental health or substance use populations preferred.

Responsibilities

  • Working to secure Medicaid and other relevant benefits.
  • Interviewing and screening for individuals needs and program eligibility.
  • Assisting individuals in maximizing community integration and normalization.
  • Providing leadership in the coordination of resources.
  • Referral of individuals to community and social support services.
  • Transitional Care and linkage, including follow-up from inpatient and other settings.
  • Provide support in navigating complex health care and social service systems.
  • Provide individuals and families support toward goals.
  • Provide advocacy for needed services.
  • Provide on-site crisis intervention and skill teaching when other services are not available.
  • Monitor and follow up contacts and other activities (at least one annual monitoring) to ensure services are consistent with the individuals’ treatment plans and adequately addressing individuals’ needs and wellness recovery goals.
  • Monitor progress by collaborating with the individual, the individual’s family or collaterals, service providers, and other relevant entities or individuals.
  • Consulting with collaterals, with clients’ permission, with the goal of improving the individuals’ outcomes while assisting them in addressing their goals.
  • Taking the lead role when collaborating with service providers in a coordinated effort to ensure continuity of care for the individual.
  • Preparing and maintaining consumer records in accordance with departmental procedures.
  • Developing other services to meet clients’ needs when no appropriate services exists, or when existing services are inadequate.
  • Reporting to supervisor/manager/director for all emergencies and other concerns and work toward a resolution.
  • Other duties as assigned to meet the needs of the consumers and department.
  • Adhere to agencies policies and procedures.
