Targeted Case Manager
About The Position
Helio Health is hiring a Targeted Case Manager to support its mission of promoting recovery from substance use and mental health disorders and other health issues. The role manages and coordinates services for patients and residents participating in or who have completed Helio Health treatment programs. This is a full-time position at the Insights of Helio Health Certified Community Behavioral Health Clinic (CCBHC). The schedule is flexible within the clinic’s operating hours: Monday through Thursday, 8:00 a.m. to 8:30 p.m., and Friday through Sunday, 8:00 a.m. to 4:30 p.m. Some evening hours are required. Compensation is $24.00 to $26.00 per hour. Full-time hires are eligible for a sign-on incentive of up to $1,500.
Requirements
- Associate's degree in a related human services field required.
- Two years of experience working with behavioral health and/or substance abuse issues.
Nice To Haves
- Bachelor's degree preferred.
- Valid New York State driver's license preferred.
Responsibilities
- Maintain a caseload and perform primary casework responsibilities, including screening, intake, orientation, referrals, counseling, crisis intervention, patient/resident education, case management, reporting and record keeping, consultation with other professionals, coordination of care with other community providers, and discharge planning.
- Train staff, volunteers, and students/interns.
- Perform quality assurance activities.
- Provide services in accordance with agency policies, procedures, program guidelines, and governmental regulations.
- Monitor provision of services when assigned.
- Evaluate programs and identify strengths and weaknesses.
- Assist in problem resolution and implementation of supervisor-approved program modifications.
- Participate in program administrative and clinical committees as assigned.
- Represent the agency to the community.
- Perform other duties as assigned by the supervisor.
