About The Position

Mending Point Behavioral Health, a program under The Methodist Home of Kentucky umbrella, is seeking a Targeted Case Manager (TCM) who is highly motivated to make a difference in the community and committed to the well-being of their clients. The TCM will utilize experience and knowledge to assess needs of clients and their families in order to link and coordinate the appropriate resources. They are responsible for managing an assigned caseload and responding appropriately to client needs in compliance with Medicaid standards. This includes but is not limited to assessing client needs; developing, implementing and reviewing service plans; and working with other community resources in achieving client service needs. Caseloads may include diverse populations with a variety of clinical needs. The Targeted Case Manager is a positive team player that promotes a culture of inclusion and participation. This is a new and developing position. We anticipate that we will continue to grow into several counties in the Northern Kentucky area.

Requirements

• Bachelor’s Degree from an accredited institution in the human services field.
• Minimum of 1 year of experience in a behavioral health setting post-bachelor’s degree.
• Twelve (12) hour Targeted Case Management Core Training
• Six (6) hour Substance Use Disorder Targeted Case Management Training
• Six (6) hour Severe Emotional Disability Targeted Case Management Training
• Six (6) hour Severe Mental Illness Targeted Case Management Training
• Must be at least 21 years of age.
• Valid driver’s license, reliable transportation, and proof of current auto insurance.
• Must submit to required background checks, fingerprinting and initial drug screen.

Responsibilities

• Assess needs of clients and their families in order to link and coordinate the appropriate resources.
• Manage an assigned caseload and respond appropriately to client needs in compliance with Medicaid standards.
• Develop, implement and review service plans.
• Work with other community resources in achieving client service needs.
• Act as the face of the Agency/Program with referring partners, clients and families.
• Follow through with documentation, communication and deadlines.
