About The Position

This position is accountable for the provision of quality outcome-based educational programs and practice support, which promote initial and ongoing competency and continuous development of Case Management staff. The role involves developing, organizing, implementing, and evaluating facility and system-based education and programs to ensure the competencies of care management personnel. It also requires promoting best practices in care coordination and facilitating orientation by collaborating with core departments.

Requirements

  • Baccalaureate Degree in Nursing Required
  • Minimum 5 years' experience in Case Management or related field.
  • Excellent communication skills
  • Understanding of human learning theory
  • Understanding of healthcare systems
  • Understanding of care management
  • Ability to manage multiple priorities
  • Working knowledge and experience with IT systems
  • Ability to utilize lean thinking and process improvement cycles
  • Ability to understand and use data
  • Current Connecticut State RN Nursing Licensure and or eligibility for Connecticut State RN licensure required.

Nice To Haves

  • Master's Degree Preferred
  • Case Management Certification desired.

Responsibilities

  • Develops, organizes, implements and evaluates Facility and System Based education and programs to ensure competencies of care management personnel.
  • Demonstrates knowledge of the American Nurses Association Standards for Nursing Professional Development.
  • Use multiple approaches and sources of information to identify and validate personnel’s learning needs.
  • Conducts learning needs assessments regularly to identify organizational and departmental education and practice needs.
  • Develops educational offering based on organization/clinical priorities and the learning needs of the intended audience.
  • Demonstrates knowledge of the financial resources available and the impact on program design.
  • Identifies organizational, service line and external agency requirements for documentation and record keeping.
  • Promotes best practice in care coordination.
  • Assists in the identification of staff for preceptor development.
  • Facilitates orientation by collaborating with the core department.
  • Integrates performance indicators in education activities.
  • Seeks out opportunities for professional growth through organizational affiliations or certification within specialty area.
