About The Position

The Supportive Living Care Manager provides recovery-oriented support and case management to adults experiencing substance use disorders, mental health challenges, and housing instability. This position works directly with residents in supportive living homes and in the community, helping them maintain stability, connect with resources, build independent-living skills, and work toward long-term recovery and self-sufficiency. This is a hands-on position for someone who is compassionate, dependable, organized, and comfortable working independently. It can be a great opportunity for someone newer to the behavioral health field who wants to build experience while making a meaningful difference in the lives of others.

Requirements

  • Interest in working with individuals experiencing substance use disorders, mental health challenges, or housing instability.
  • CPSS (Certified Peer Support Specialist), case management credential, or eligibility to obtain an applicable credential is required.
  • If using personal recovery experience as a qualification or providing peer services, at least one year of stable recovery is required.
  • Valid driver's license, reliable transportation, and ability to meet organizational driving requirements.
  • Strong communication, organization, and basic computer skills.
  • Ability to maintain professional boundaries, confidentiality, and appropriate documentation.
  • Must be dependable, self-motivated, willing to learn, and able to work both independently and as part of a team.
  • Must meet applicable background screening and program requirements.

Nice To Haves

  • Experience in behavioral health, recovery services, peer support, case management, or related work is preferred but not required.

Responsibilities

  • Build supportive, professional relationships with residents and provide individualized case management and recovery support.
  • Help residents work toward recovery, housing, treatment, employment, financial, and independent-living goals.
  • Connect residents with behavioral health providers, healthcare, benefits, housing resources, and other community services.
  • Assist residents with maintaining housing and preparing for successful transitions to greater independence.
  • Provide support during crises and coordinate with supervisors, treatment providers, and other appropriate resources.
  • Maintain accurate and timely documentation of services and resident progress.
  • Provide approved transportation to appointments and other necessary community activities.
  • Support the safety, cleanliness, and overall stability of assigned supportive living residences.
  • Communicate and coordinate with treatment providers, community partners, house leadership, and other members of the resident's support team.
  • Manage scheduled and unscheduled work time responsibly and follow through on assigned responsibilities.
