Every Connection Opens a Door to Care. Valley Access Supportive Living serves as a vital bridge between individuals seeking help and the services that can support their journey. As an Access Coordinator, you'll guide clients through the referral, screening, and intake process while connecting them with the resources they need to move forward. Through thoughtful coordination, compassionate communication, and strong community partnerships, you'll help ensure individuals receive timely access to care and support.

Requirements

  • High school diploma or equivalent
  • None (experience)
  • Case Manager certification
  • CPR certification
  • Valley de-escalation certification
  • Driving positions require a minimum age of 21 and a current driver's license

  • Bachelor's degree in social work or related field
  • Previous social services experience

Responsibilities

  • Manages incoming referrals through telephone calls, emails, and other contact types.
  • Gathers required information from referral sources.
  • Completes eligibility and program screening with referred clients; schedules intakes and follow-up appointments.
  • Verifies insurance information and discusses funding options with clients.
  • Coordinates with program staff to ensure completion of registration paperwork and any necessary documentation.
  • Tracks program capacity and intake availability; manages program waitlists as appropriate.
  • Coordinates care by connecting referred clients to alternative treatment options and community partners as appropriate.
  • Participates in community outreach, engagement, and education regarding Valley services and the referral and intake process.
  • Tracks key access information including no-show rates, hospitalizations and follow-up, court tracking/reporting, and overall program and client engagement levels.
  • Ensures that grant-related, state and local, and payer access requirements are met.
  • Follows agency documentation policies for all phases of treatment, including assessments, care plans, treatment transitions and discharge plans, and client care.
  • Collaborates with internal and external multidisciplinary teams regarding clinical aspects of treatment.
  • Adheres to ethical and legal standards set by licensing boards and state regulations.
  • Meets direct care expectations.
  • Participates in ongoing professional development activities to meet licensure, certification, and/or other agency requirements.
