About The Position

Favarh is seeking an independent, compassionate, creative and organized Supportive Housing Manager (SHM) to lead our inclusive supportive housing apartments in Avon, CT. In this role, you will work closely with our Supportive Housing Director and Supportive Housing Coordinator to oversee operations, supervise staff, coordinate individualized supports (ADLs, transportation, employment), and ensure the success of this community-based housing for adults with intellectual and developmental disabilities (I/DD) while adhering to DDS policies and Favarh standards. This is one of Favarh’s inclusive housing developments, offering integrated, person-centered living options in the heart of Farmington Valley and surrounding towns.

Requirements

  • Bachelor’s degree preferred or equivalent experience in human services, residential, or IDD-related field
  • Minimum 3 years of supervisory experience in a residential or supportive housing setting
  • Medication Administration Certification (required within 3 months of hire)
  • Certifications in PMT, CPR/First Aid/AED and DDS Medication Administration Certification (or must obtain within 60 days)
  • Familiarity with DDS regulations, EVV, Therap, Microsoft Office, Excel and electronic scheduling systems
  • Strong organizational, interpersonal, and communication skills
  • Valid Connecticut Driver’s License with a clean driving record (REQUIRED)
  • Must meet Favarh transport driver approval (Ride Share eligibility)

Responsibilities

  • Oversee daily operations of the Canton Supportive Housing program
  • Coordinate & Oversee tenant move-ins, orientation, and transition planning
  • Maintain 24-hour on-call availability per Emergency On-Call Procedures
  • Ensure apartment conditions, safety standards, and personal care supplies are maintained
  • Assist all individuals in the art of living in their own apartment and being a good tenant.
  • Create and manage individualized schedules for ADLs, transportation, employment, recreation, and medical appointments
  • Provide life-skills coaching and individualized support; refer clinical concerns as needed
  • Ensure timely completion of medical, psychiatric, and dental appointments according to DDS guidelines and individuals health and medical needs
  • Coordinate and maintain participant records in compliance with DDS standards
  • Assist in habilitation and case coordination activities as needed
  • Assist individuals with respect to financial oversight, including budgeting, bill paying and redeterminations as assigned.
  • Directly supervise Supportive Housing Assistant Manager, Medical Liaison, and DSPs
  • Participate in hiring, training, scheduling, and performance management of staff
  • Provide ongoing training and “on-the-floor” coaching in accordance with Favarh and DDS Quality Assurance guidelines
  • Ensure staff compliance with Medication Administration, First Aid/CPR, PMT, and all mandated training courses
  • Utilize & monitor documentation in Therap to ensure complete and accurate billing (EVV), service delivery, and daily logs
  • Monitor participant finances and ensure spending aligns with approved budgets
  • Participate in PRC/HRC process, reviews, audits, and other regulatory processes as assigned
  • Maintain program documentation, safety protocols, and purchasing in line with agency policy and DDS regulations
  • Monitor use of agency vehicles and ensure driver compliance with Favarh policy
  • Ensure EVV documentation aligns with Therap records
  • Manager is responsible for ensuring that documentation within Therap and in personal (hard copy) files is complete, accurate, and maintained to meet the requirements of the (DDS) licensing and Quality Service Review (QSR).
  • Maintain regular communication with families/guardians, team members, internal departments and external service providers
  • Coordinate participant and family orientation to the program
  • Act as liaison between families, healthcare providers, neighbors, local groups, and the broader community to promote inclusion and awareness
  • Support participant engagement and integration into the local community through social and recreational planning
  • Promote Favarh’s mission and values to neighbors, stakeholders, and the public
